Diabetes Care

The American Diabetes Association recognizes this education service as meeting the national standards for diabetes self-management education.

TriStar Centennial’s Outpatient Diabetes Education Program has been recognized by the American Diabetes Association for Quality Self-Management Education. This ensures the delivery of high quality care to all patients. Our Certified Diabetes Educators (CDE's) and Registered Dietitians (RD's) are here to help you take control and balance your life with diabetes. We work with your physician to develop an effective treatment plan to meet your specific needs and lifestyle. Our RD's see patients with any diagnosis that requires special diets or patients desiring weight loss.

Learning to live with diabetes is the best approach to minimizing the complications while maximizing the quality of your life. Our goal is to help you learn the skills to cope and live successfully with diabetes.

Please call the TriStar Centennial Diabetes Center at (615) 342-3974 to get more information about our programs or to schedule an appointment.

A physician order is required for our services. In most cases, diabetes education is covered by your health insurance. Check with your insurance provider for details.
